Reporting a Defect
A defect is a problem found on an asset that needs to be tracked and resolved. Defects can be created manually or generated automatically from failed checks.
Automatic defect creation
When a check template has auto-create defects enabled, a defect is raised immediately whenever a check fails. The auto-created defect:
- Is linked to the failed check instance
- Uses the severity defined in the template's fail logic
- Has a title like "Failed check: [Template Name]"
- Starts in Open status
- If the severity is Critical and force-OOS is enabled, the asset is automatically taken Out of Service
Manual defect creation
Anyone with the right permissions can report a defect manually:
- Go to Defects and click Report Defect, or click Report Defect on the asset's page
- Fill in the details:

Severity levels
Choose the severity based on the impact of the defect:
| Severity | When to use |
|---|---|
| Critical | Immediate danger to life or risk of serious injury. Equipment must not be used. |
| High | Significant safety risk or major operational impact. Needs urgent attention. |
| Medium | Moderate concern that should be addressed in a reasonable timeframe. |
| Low | Minor issue with no immediate safety impact. Can be scheduled for routine maintenance. |
| Info | Informational observation — no action required but worth recording. |
Isolating an asset
When reporting a critical defect, you may need to isolate the asset — taking it Out of Service so no one uses it until the defect is resolved. You can flag the defect as isolated out of service, which:
- Records that the asset was taken out of service because of this defect
- Automatically restores the asset to In Service when the defect is closed and verified
